

Gothic Arc recording device
AFR-Mini-Reg is the device previously designed as a JIG-Splint. It can be used in patients with complete dentition or in patients with missing teeth or spaces in the posterior area. It is an economical and effective device for the diagnosis of malocclusion and determination of a physiologic position of the mandible.
AFR-MINI-Reg has been used in professional dentistry for almost 30 years.

Indications
AFR-Mini-Reg is an instrument for recording the exact location of the centric and eccentric relationship of the mandible. It can be used before, during and after treatment (occlusal splint, orthodontic cases, orthognathic cases, selective grinding, restorative dentistry, prosthodontics). It is also recommended for patients with implants. AFR-Mini-Reg helps establish a prominent mandibular position for snoring and apnea appliances. Also, for orthodontic treatment using a Herbst-Appliance appliance, mandible positions can be registered with an AFR-Mini-Reg. Just like any other JIG-Splint (Lucia-JIG, NTI-TSS, Leaf-Gauge, Denar Deprogrammer, Kois Deprogrammer, ...) the AFR-Mini-Reg is a frontal deprogrammer, using the Gothic Arch as a system comparable to Gysi, Gerber
Based on clinical experience, the AFR-Mini-Reg is an additional device, useful when using electronic recording systems such as Jaw Motion Analyzer, Arcus Digma, Axioqick Recorder, Freecorder BlueFox.
How to apply AFR-Mini-Reg to anterior teeth
Silicone PVS (putty) supports all cases; however, it is recommended to apply the adhesive beforehand (Fig.3, Fig.4). The use of a primer confers better retention.
Green sticks (mixing sticks) can also be used (Fig. 5), with restrictions for cases with retention (fixed prosthesis, braces) or even in the presence of temporary Bis-Acryl-Composite crowns and bridges.
All types of wax or other materials are not indicated for this technique.
Mini-Reg stability procedure
Whether silicone or blending sticks, the MINI-REG must be held stationary during alignment without rotational movement. (Fig.6) The MINI-REG impression material must cover the teeth and the anterior palatal region of the gum.
What is a Gothic arch?
Gothic arch registration is a technique that has been used in dentistry for more than 100 years. Alfred Gysi used an extraoral device to record mandibular movements for full denture reconstruction.
The Gothic Tracer instrument is used to determine the position of the mandible in centric relation. The traces represent the movement of the mandible on a horizontal plane. The apex of the arch considers the mandible to be in centric relation. The traces show the maximum movement of the mandible in anterior and lateral position.
What is the difference between AFR-Mini-Reg and other common systems
Almost all gothic arch tracking systems (Gerber, DIR-System, ...) are bite plates that are mounted on the intraoral position of the mandible. The bite plate of the AFR-Mini-Reg appliance has a flat surface that is mounted on the anterior teeth. The utility of this device is more convenient for both the dentist and the patient.
Application of the AFR-Mini-Reg is quick and easy. A reliable and accurate diagnosis can be made in less than 15 minutes.
The AFR-Mini-Reg is also an alternative for patients on a budget.
Additional products needed for gothic arch registration
Gothic arch registration can be done directly on the bite plate using an occlusion spray. (Fig.8) The surface should be evenly coated and smooth with no spots. Using this technique, good results can be obtained by using Bausch Spray BK 285 white (Fig.7) Other colors such as green can also be used. White spray provides good contrast on the blue surface. In addition to the direct application, a joint paper is added to each AFR-Mini-Reg. This articulation paper is a self-adhesive label, which fits exactly to the shape of the bite plate.
To prepare a record, the dentist takes a sheet of occlusion testing film on one side (Bausch Arti-Fol Metallic) (Fig. 9) and places the labeling portion on the top of the articulation paper. The unmarked side is exposed to the anterior mandibular registration pins. The marks on the articulation paper (Figs. 10, 12, 14) are clearly visible, and the paper should be sealed with transparent plastic labels, which is also attached to the AFR-Min-Reg. These articulation papers can be used for documentation purposes by adding them to the patient record.
Evaluation of Gothic arch records
Using articulating paper as documentation can provide important information about the maximum movement of the mandible. It may also be useful to keep such records before and after treatment, with occlusal splint, orthodontic treatment, temporary crowns, ..., to compare the results and to check for any improvement. The millimeter scale (with the kit) has the advantage (with BK 30 black, BK 31 red, Fig.15) to compare the range of mandibular movements, before and after occlusal splints, orthodontic treatment, temporary crowns.
INTER-OCCLUSAL RECORDING - What are the characteristics of an inter-occlusal record.
You can use wax, modeling resin and silicone (Fig.16). It is important that the interocclusal registration shows only peak marks and allows optimal adjustment with the diagnostic mold. High quality materials give better results; however, training in mandibular manipulation is necessary to obtain good results.
CAUTION: Accurate inter-occlusal recordings require your utmost attention to obtain an inter-occlusal recording result that exactly matches the diagnostic (Fig. 17).

The AFR-Mini-Reg is the only device that incorporates the advantages of an anterior jig splint and a gothic arch registration. By supporting neuromuscular relaxation, the AFR-Mini-Reg allows the mandible to sit in a physiologic position. (Fig.18).
It is also possible to determine an eccentric mandibular position with an inter-occlusal registration. (Fig. 19, 20, 21) The AFR-Mini-Reg is like a Jig-Splint, which enhances a neuromuscular relaxation, manual manipulation of the mandible is in most cases not necessary. However, it is important that the dentist has clinical experience for those cases, which probably require manual support.
Gothic arch registration with a class III occlusion angle
The AFR-Mini-Reg can also be used to diagnose a class III angle. In this particular case, the bite plate is mounted on the lower anterior lower teeth and the registration pin on the anterior front teeth to increase the range of movement on the bite plate. In this case, the tip of the Gothic arch registration is in an inverted position. (Fig.22). For an individual with a class III condition with a larger mandibular prominence, the MINI-REG device parts should be inverted so that the gothic arch remains narrow on the biteplate. Thus it is more stable for a class III condition.
INTER-OCLUSAL REGISTRATIONS - Mounting the lower mold in the articulator
Only the interocclusal registration is required for mounting the lower casting model.
It is not recommended to also use the AFR-Mini-Reg when mounting the models, as the positioning of the AFR-Mini-Reg on the stone models differs from that in the mouth. The inter-occlusal registration is placed on the cast model of the jawbone. (Fig.25)
The lower cast model is placed in the impression of the inter-occlusal registration. At this stage, the incisor stift of the articulator is fixed to adjust the vertical dimension (Fig.23). Depending on the model of the articulator used, different adjustments must be considered according to the requirements of the technical specifications.
Transferring the mandibular position using interocclusal registration provides an accurate reproducible position in the articulator. In order to obtain reproducible and accurate positions, it is essential to consider possible sources of error:
- Veneer transfer
- Dimensional changes of the impression and consequently of the molded model
- Dimensional changes of the inter-occlusal registration
- Diagnostic errors on the cast pattern
To check the concordance of occlusal contacts between the cast model and the intraoral situation, the Arti-Dry technique is a technique that can be used, but requires clinical experience of manual manipulation of the mandible. In this case, the mandible must be manually placed in CR (Centric Relation).
If the occlusal interferences are in concordance, the molded model is correctly fitted. To verify this concordance, films or articulating paper can be used. (Fig.27)
DIAGNOSTIC INTERPRETATION - SPECIFIC INTERVENTIONS
Occlusal space
The AFR-Mini-Reg device creates an occlusal space between the maxilla and mandible. Like any other Jig-Splint (Lucia-JIG, NTITSS, Leaf-Gauge, Denar Deprogrammer, Kois Deprogrammer, ...), the AFR-Mini-Reg generates a rotation of the condyle. Based on clinical experience, an anterior occlusal space of maximum 12 mm has no significant effect in determining the joint axis (Fig. 30), as long as there is no condylar translation. This finding has also been mentioned in the relevant literature (McCollum, Posselt, Ramjford & Ash, Howat, Okenson, Dawson, Santos Jr., Bumann). In general, the AFR-Mini-Reg creates an interincisal opening of 7-9 mm. (fig. 28, 29)
Reference plane
In order to realize an AFR-Mini-Reg registration, it is recommended to attach the biteplate relative to the occlusal plane. Small deviations from the occlusal plane did not show different results. (Fig. .31, 32)
Equivalent recording techniques
In addition to the AFR-Mini-Reg technique (Fig. 33) there are numerous techniques and concepts, which lead to the same result, such as Lucia-JIG (Fig. 34), Dawson Bimanual Manipulation (Fig. 35), ....
Eccentric inter-occlusal recordings
AFR-Mini-Reg is unique in that it is a combination of a neuromuscular anterior deprogrammer and a gothic arch registration. One of the major advantages is the possibility to also make eccentric inter-occlusal recordings. (Fig.36, 37, 38)
Splint Oclusal
In dentistry there are numerous techniques or concepts to make an occlusal splint. The AFR-Mini-Reg is a practicable device, transferring diagnostic information into an individual hard splint for the patient. (Fig. 39, 40)
Occlusal splint in physiologic position with guided canine occlusion. Acting like any anterior deprogrammer, the AFR-Mini-REG supports the patient's neuromuscular system. The anterior deprogrammer is very comfortable during the registration process, relaxing the masticatory muscles.
Eccentric recording
The AFR-Mini-Reg can also be used with electronic jaw motion systems such as Jaw Motion Analyzer/Zebris, Arcus Digma/Kawo, Axioquick Recorder/SAM, Freecorder BlueFox .... (Fig. 41, 42, 43, 44)
Orthodontic treatment
Patients with braces can also have an AFR-Mini-Reg. (Fig.45) For this technique, the lower registration appliance must be adjusted.
Patients with movement problems
If the patient has a limitation of movement to the right or to the left, the problem is directly visible following the Gothic Arch recording. This limitation could be caused by intense muscle tension, displacement of the disk without reduction or degenerative joint disease (Fig. 46).
Additional products
When a patient consistently experiences muscle pain, an Aqualizer can be used before starting a recording. Both hydrostatic systems will significantly reduce muscle pain. The patient should use this system one week prior to the recording procedure.
